About the Pennsylvania Institute for Community Living (PICL)
The Pennsylvania Institute for Community Living (PICL) is a 15-bed residential program within RHD that offers integrated self-help, recovery-oriented treatment, and psychiatric rehabilitation. PICL provides 24-hour staffing and assists residents in securing permanent housing. The program focuses on functional skill development through structured lesson plans and group activities covering areas such as social skills, vocational skills, tenancy management (budgeting and financial planning), time management, emotional expression, and coping mechanisms.
Position Summary
As a Clinical Care Coordinator, you will play a vital role in organizing patient care activities and facilitating communication among all stakeholders involved in a patient's treatment. Your primary responsibility is to ensure safe and effective treatment outcomes by coordinating care across various settings.
Essential Duties and Responsibilities
- Develop and monitor program initiatives, ensuring alignment with funder requirements, community support services, government licensing agencies, and other relevant resources.
- Build and maintain strong relationships with program participants, RHD staff, and external providers to coordinate comprehensive care.
- Maintain a thorough understanding of the services offered throughout the RHD service line.
- Conduct regular reviews of each client's care plan, ensuring they are connected with all available resources and receive appropriate support.
- Develop comprehensive treatment plans for clients requiring multiple services across different settings.
- Review compliance of individual treatment plans and maintain a tracking system to identify the need for comprehensive plans.
- Collaborate with in-house and external partner providers through regular off-site visits.
- Facilitate routine comprehensive treatment team meetings to ensure collaborative planning and care coordination.
- Provide statistical data and outcome/performance measures on the quality of care delivered.
- Prepare weekly and monthly reports as required.
Qualifications
- Master's Degree in a Human Service field.
- One to two years of experience working with children and/or adults with mental health diagnoses, intellectual or developmental disabilities, or substance use disorders.
- Valid Driver's License.
