Key Responsibilities- Develop and implement comprehensive care plans that integrate patient and family preferences and values.
- Monitor patient care through regular assessments, evaluations, and record reviews.
- Advocate for resources and removal of barriers to ensure effective care delivery.
- Maintain ongoing communication with supervisors and healthcare providers to ensure seamless care coordination.
- Ensure accurate and timely medical record documentation and treatment planning.
- Licensure/Certification: One or more of the following required: Clinical Counselor, Counselor, Social Worker (Clinical Practice), or Social Worker.
- Education: High School diploma equivalency with 2 years of cumulative experience, Associate's degree, or 4 years of applicable cumulative job-specific experience.
