Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Trinity Health Senior Communities. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct thorough assessments, identify patient needs, and prioritize care accordingly.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to a culture of excellence.
  • Collaborate with interdisciplinary teams to achieve optimal patient outcomes.
  • Document patient information accurately and thoroughly in accordance with regulatory requirements.
  • Maintain a safe and clean environment, adhering to infection control and safety protocols.
  • Participate in ongoing education and professional development to enhance skills and knowledge.
Requirements
  • Current LPN licensure in the state of employment.
  • Graduate of an accredited LPN program.
  • Basic Life Support (BLS) certification.
  • Previous experience in long-term care or geriatric settings preferred.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Flexibility to adapt to changing situations and priorities.
