Job Summary

The Care Case Manager will be responsible for creating personalized care plans, managing care services, and providing exceptional communication and problem-solving skills to our clients, their representatives, and caregivers. This is a part-time or full-time position, with a preference for working weekends.

Key Responsibilities

  • Create unique care plans tailored to each client's needs
  • Manage care services, including coordination with caregivers and office staff

  • Work closely with our office team to ensure seamless care delivery
  • Develop and maintain strong relationships with clients, caregivers, and community partners

Requirements

  • Master's degree in Social Work or a relevant field (preferred)
  • Industry experience (preferred)
  • Strong computer skills, including proficiency in care management software
  • Excellent organizational and communication skills
  • Ability to pass Live Scan Fingerprint Clearance (LIC 9163)
  • Valid driver's license and proof of insurance
  • Cell phone with internet access
  • Proof of negative TB results
