Job Summary

Nile Sisters Development Initiative (NSDI) is seeking a part-time Licensed Clinical Social Worker to provide case management and direct services to patients. The successful candidate will have a Master's degree in social work or a related field and at least one year of experience in case management and direct services.

Key Responsibilities
  • Provide case management and direct services to patients, including assessments, treatment plans, and discharge planning.
  • Facilitate emotional support workshops, crisis intervention, and support groups.
  • Conduct behavioral health assessments and determine mental health and social service needs of program participants.
  • Address referrals from providers and coordinate services.
  • Assess the holistic and programmatic needs of patients and coordinate case management.
  • Work collaboratively with multidisciplinary team members and external partners.
  • Develop program materials, forms, and tracking measures.
  • Accurately maintain electronic files and record data in state and internal databases.
  • Practice excellence in customer service and provide culturally proficient and responsive services.
  • Conduct presentations to promote programs to various outlets.
Requirements
  • Master's degree in social work or a related field.
  • Current license as a Licensed Clinical Social Worker (LCSW) and National Provider Identification Number (NPI).
  • Possess or be willing to complete HIPAA training.
  • Ability to collect and interpret qualitative data and prepare reports.
  • Effective oral and written communication skills.
  • Ability to work effectively with limited supervision.
Working Conditions
  • Part-time - 20 hours/week.
  • Flexible work environment, with the ability to work in an office setting and use electronic equipment.
  • Flexibility in schedule, including scheduled weekends when necessary.
  • Visual/hearing ability sufficient to comprehend written/verbal and telephone communication.
