About the Role:

This is a full-time position that requires a high level of customer service and a commitment to delivering exceptional care to patients with life-limiting illnesses. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities:

  • Direct Patient Care: Provide hands-on care to patients under the supervision of a Registered Nurse, including assistance with daily living activities, bathing, dressing, and grooming.
  • Care Planning: Participate in the development and implementation of individualized care plans that meet the unique needs of each patient.
  • Communication: Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and coordinate services as needed.
  • Patient Education: Educate patients and their families on disease management, treatment options, and wellness strategies to promote optimal health outcomes.

Requirements:

  • Certification: Current Certified Nursing Assistant (CNA) license in the state of employment.
  • Experience: At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and demonstrate empathy and compassion when interacting with patients and families.
