Job Summary:

Northern Cardinal Rehabilitation and Nursing is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care and lead our nursing team. As an LPN, you will be responsible for delivering direct nursing care to our residents, supervising nursing assistants, and maintaining the highest standards of quality care.

Key Responsibilities:

  • Provide direct nursing care to residents, including administering medications, wound care, and vital sign monitoring.
  • Supervise and delegate tasks to nursing assistants, ensuring that care is delivered in accordance with facility policies and procedures.
  • Conduct regular assessments of residents' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with the interdisciplinary team to coordinate care and services, ensuring that residents receive comprehensive and coordinated care.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance the quality of care.

Requirements:

  • Current state license as a Licensed Practical Nurse (LPN).
  • Current/active CPR certification.
  • Excellent technical, assessment, and documentation skills.
  • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity.
  • Strong analytical and problem-solving skills.
  • Ability to lift 50 pounds or more, with physical work being a primary part of the job.
  • Able to stand and/or walk for more than four hours per day, bend and/or stoop for more than one hour at a time.
