Position Overview
The Aftercare Case Manager plays a crucial role in our Streetwork Project, an innovative outreach and counseling initiative for homeless youth in urban areas. Utilizing a harm reduction strategy, this position provides comprehensive support to adolescents and young adults affected by homelessness and engaged in street activities.

The Aftercare Case Manager will assist youth in accessing essential resources, focusing on housing and benefits navigation.

Key Responsibilities:
  • Implement a trauma-informed, client-centered approach in all interactions.
  • Engage adolescents in program activities, fostering a respectful environment.
  • Provide milieu counseling and mediate internal conflicts.
  • Facilitate access to daily necessities such as food, clothing, and hygiene supplies.
  • Coordinate services with Streetwork staff and on-site service providers.
  • Assist youth in applying for supportive and permanent housing options.
  • Conduct intake assessments to determine housing eligibility.
  • Connect clients with Housing Navigators and Benefits Coordinators, providing necessary support.
  • Advocate for clients within the shelter system and other social service programs.
  • Prepare case records and statistical reports as required.
  • Support youth in maintaining housing stability post-shelter.
  • Facilitate presentations on aftercare and independent living skills.
  • Participate in case conferencing and clinical group processes.
  • Manage routine administrative tasks, including correspondence and file maintenance.
  • Engage in staff training and meetings.
Qualifications:
  • High school diploma or equivalent, with relevant training and experience in human services.
  • At least one year of experience working with homeless populations or youth.
  • Strong communication skills and a compassionate approach to client interactions.
  • Basic knowledge of NYC housing resources.
  • Ability to work independently and manage multiple tasks effectively.
