Job Summary

The Center for Urban Community Services (CUCS) is seeking a skilled Case Manager to join our Street Outreach Program. As a key member of our interdisciplinary team, you will provide direct services to clients living on the streets and work towards achieving permanent housing goals.

Key Responsibilities
  • Provide outreach services to clients living on the streets, including assessing their needs and developing individualized plans.
  • Assist clients in obtaining permanent housing, including connecting them with available resources and supporting them through the housing application process.
  • Work collaboratively with our team to ensure the successful reintegration of clients into their communities.
  • Meet contractual obligations, including achieving housing placement goals and contributing to the overall success of the program.
Requirements
  • Valid driver's license and clean driving record.
  • Relevant experience working with homeless populations, with a preference for those with Spanish language proficiency.
  • High school diploma and 2 years of experience, or equivalent combination of education and experience.
  • Demonstrated ability to provide services to a specialized population, with strong computer literacy and communication skills.
  • Ability to work effectively as part of a multidisciplinary team and adapt to changing program needs.
