Job Summary
A challenging and rewarding opportunity has arisen for a skilled Nurse Practitioner to join our team at LiveWell Choice, LLC. As an Advanced Care Coordinator, you will play a vital role in delivering high-quality healthcare services to seniors in long-term care settings.

We are seeking a dedicated and compassionate individual with experience in gerontological nursing to provide collaborative primary care and services to a panel of members. The ideal candidate will possess excellent clinical skills, strong communication and interpersonal abilities, and the ability to work effectively in a team environment.

This is a full-time position that requires a Master's Degree in Nursing as a Nurse Practitioner from a nationally accredited program, or a Master's Degree from a nationally accredited program. Additionally, national board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner is required. A DEA License, active national provider number (NPI), and active Medicare provider number are also essential qualifications for this role.
