Position Overview

This role is designed as a flexible, part-time position. We welcome your availability and will work to accommodate it.

Key Responsibilities of the Hospice LPN:

  • Report directly to the Administrator or Executive Director.
  • Deliver direct patient care services to hospice patients and provide supportive care to their families, managing a reasonable caseload.
  • Assess patient and family needs, assisting in the development and implementation of individualized care plans.
  • Communicate any changes in patient conditions to the Registered Nurse, interdisciplinary team, and referring physician, ensuring thorough documentation.
  • Participate in staff meetings to enhance team collaboration.

Qualifications

Essential attributes for a Hospice LPN:

  • A compassionate heart, demonstrating kindness and empathy towards patients and their families.
  • Comfort in providing care in various home settings.
  • Ability to maintain composure and flexibility while supporting others during challenging times.
  • Valid Licensed Practical Nurse (LPN) license in the state of employment.
  • A minimum of 1 year of patient care experience in hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.
  • Additional preferred experience includes roles such as hospital Staff LPN, Staff LVN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel nursing.
  • A current driver's license and automobile insurance.
  • Valid CPR certification.
