Job Overview:
We are seeking a compassionate and skilled Hospice LPN to join our team. As a Hospice LPN, you will provide direct patient care services, evaluate patient needs, and assist in the preparation and execution of the plan of care. You will also report findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, and document all reports.
Responsibilities:
Provide direct patient care services to hospice patients and supportive care to the patients' families with a reasonable case load.
Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient.
Report findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, and document all reports.
Attend staff meetings.
Requirements:
We are looking for a Hospice LPN with a compassionate heart, kind and empathetic to patients and their families. You should be comfortable providing care wherever the patient calls home, and demonstrate flexibility and composure while supporting others in a time of crisis. You must have a Licensed Practical Nurse / LPN license to practice in the state of requested employment, and 1+ years of patient care experience in hospice, home health, ICU, geriatric, or oncology. Current drivers license and automobile insurance, and current CPR certification are also required.
