Job Title: Trach and Vent Private Duty Nurse LPN

Aveanna Healthcare is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the home setting. As a Trach and Vent Private Duty Nurse LPN, you will be responsible for delivering high-quality patient care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.

Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, and report changes in condition to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
