About the Role

We are seeking a highly skilled Physician Assistant to join our team as a Primary Care Specialist. This role provides medical care and treatment services with an emphasis on chronic disease management and promotion of health and palliative care for patients not best served in a clinic setting. The ideal candidate will have a strong background in geriatrics and palliative care, with excellent communication and interpersonal skills.

Key Responsibilities

  • Provide diagnosis, treatment, consultation, and follow-up care of patients under collaboration with a licensed physician.
  • Perform comprehensive health assessments, identify actual and potential health needs, and make referrals to appropriate social or medical agencies and practitioners.
  • Develop and implement proactive patient-centered care plans with the assistance of other disciplines within the care team.
  • Order testing, medications, and perform surgical procedures as appropriate under certification.
  • Approve medication refills, home health orders, and respond to caregiver requests to provide continuity of care.

Requirements

  • Master's level education in primary care program for physician assistants or related field.
  • Licensed physician assistant in the State of Oregon.
  • Certification by NCCPA.
  • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
  • CPR certification at hire or within 6 months in position.

Work Environment

This role requires frequent travel outside of the workplace, with use of a personal vehicle. A valid driver's license and acceptable driving record are essential requirements. The employee must be able to lift, carry, push, pull, stand, reach, bend, and/or pinch small objects for at least 3-6 hours/day.
