Independent Living Support Specialist

The Independent Living Support Specialist (Qualified Intellectual Disabilities Professional) plays a vital role in delivering case management services to individuals with intellectual and developmental disabilities, as well as their families, who are beneficiaries of the Medicaid Home-Based Waiver. This position is focused on facilitating, directing, and overseeing services that foster autonomy for approximately 45-50 clients.
**Key Responsibilities:**
- Engaging with Independent Service Coordination agencies for Discovery and/or Personal Planning.
- Informing individuals of their rights as mandated by regulatory bodies.
- Ensuring timely completion of Individual Support Plans, Person-Centered Plans, and assessments while maintaining document accuracy.
- Documenting monthly interactions with clients, families, and community service providers in the designated agency format.
- Completing all required documentation and paperwork within established deadlines.
- Maintaining comprehensive virtual files that comply with regulatory standards and confidentiality policies.
- Providing training in practical skills to empower individuals and families to independently manage Waiver services.
- Facilitating access to community resources and additional service providers as necessary.
- Overseeing waiver funding, managing monthly budgets, ensuring Medicaid eligibility, and maintaining compliance records.
- Assisting clients in acquiring adaptive equipment, assistive technology, and modifications for home and vehicle accessibility.
**Qualifications:**
- A minimum of one year of experience working directly with individuals with developmental disabilities.
- A Bachelor's degree in a Human Services discipline.
**Additional Requirements:**
- Exceptional verbal and written communication skills.
- Clear background checks from State Police and Health Care Worker Registry.
- A valid driver's license.
- Must be at least 21 years of age.
- Successful completion of pre-employment drug screening.
**Work Schedule:**
Monday to Friday, 8:00 AM to 4:30 PM
