Home Health Speech Therapist Case Manager

We are seeking a skilled Speech Therapist Case Manager to join our Home Health team. As a key member of our team, you will be responsible for conducting speech and language evaluations, establishing plans of care, and treating patients with various speech, language, voice, and swallowing disorders.
Key Responsibilities- Conduct thorough speech and language evaluations to identify patient needs and develop personalized plans of care.
- Evaluate patient outcomes and adjust treatment plans as needed to ensure optimal results.
- Communicate effectively with physicians, management, and other healthcare professionals to ensure seamless care coordination.
- Manage patient records and maintain accurate documentation in accordance with state and federal guidelines.
- Provide ongoing assessment and treatment of patients, including teaching and applying interventions as necessary.
- Collaborate with management to discuss caseload issues and changes in case census.
- Develop and implement plans for patient discharge, including providing written home programs, patient/family teaching, and referral for community services.
- Master's Degree in Speech Pathology.
- Speech-Language Pathology license issued by the state of Wisconsin.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association.
- Valid driver's license and reliable transportation.
- Intermediate computer skills and excellent communication and interpersonal skills.
