As a Home Health PM RN Case Manager at Advocate Aurora Health, you will be responsible for providing high-quality patient care and case management services to patients in their homes. This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities:
- Conduct thorough assessments of patients' physical, social, and environmental needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide direct patient care, including wound care, medication management, and education.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care transitions.
- Document patient information accurately and timely, including progress notes, treatment plans, and discharge summaries.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Current RN licensure in the state of practice.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
- 3+ years of experience in clinical nursing, preferably in home health or case management.
