Job Summary:

We're seeking a skilled and compassionate RN Case Manager to join our team. As a Home Care RN Case Manager, you will have the privilege of working with patients and families to develop and implement personalized care plans that promote health, independence, and happiness.

Responsibilities:
  • Complete patient plan of care and submit admission paperwork
  • Perform ongoing assessments of patients and develop a plan of care based on assessment findings
  • Identify patient goals and develop a plan to achieve them
  • Communicate with physicians, clinical managers, and interdisciplinary team members about patient status
  • Audit clinical records to ensure compliance with regulations and agency policy
