About the Role

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our team at The Neighborhood at Willow Crossing, a skilled nursing facility specializing in memory care. As an LPN, you will play a vital role in providing exceptional care to our residents.

Key Responsibilities
  • Provide direct and indirect nursing care to residents, as prescribed by physicians, ensuring care is delivered according to facility standards and policies.
  • Assess residents' needs and assist in developing and implementing individualized care plans.
  • Supervise and manage personnel on your unit, ensuring residents' needs are met and direct care staff are trained and equipped to provide quality care.
  • Coordinate nursing services with other departments, including dietary, social services, activities, and physical therapy.
  • Initiate and update nursing care plans for residents on your unit.
Requirements
  • Graduate of an accredited School of Nursing and current licensure in good standing.
  • Health certificate/examination as required by state agencies.
  • Prefer at least one year's experience in Long Term Care or a related field.
  • Ability to maintain a professional attitude, including confidentiality, concern for all people, cooperativeness, and conscientious use of time.
