Responsibilities:
- Supervise and Coordinate Care: Supervise the care and services provided by our team members, ensuring that our residents receive the highest level of care and attention.
- Medication Management: Provide safe and accurate medication-related interventions to our residents, ensuring that their medication needs are met in a timely and effective manner.
- Assessment and Planning: Assess the health and well-being of our residents, developing and implementing individualized plans of care to meet their unique needs.
- Collaboration and Communication: Work collaboratively with our interdisciplinary team to ensure that our residents receive comprehensive and coordinated care.
Qualifications:
- Education and Experience: 1-3 years of experience in a long-term care setting, with a current LPN license and CPR certification.
