About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care team as a Palliative Care Nurse Specialist. As a member of our team, you will provide expert care to patients and families in their homes, utilizing your knowledge and skills to improve quality of life.

Responsibilities
  • Provide one-to-one care with patients in their homes, prioritizing their comfort and dignity
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans
  • Conduct clinical consultations to assess patient needs and preferences
Requirements
  • Bachelor's degree in Nursing required
  • Current PA RN License and CPR Certification
  • Hospice/Palliative Care Certification preferred
