Primary Care Role

We seek an experienced Nurse Practitioner to join our team in the Greater Philadelphia region. As a Primary Care Provider, you will work under the supervision of a physician to deliver high-quality care to homebound and palliative patients. You will apply medical techniques and principles to assess patients' health, create personalized care plans, and provide ongoing support.

Your key responsibilities will include:

  • Providing comprehensive primary and palliative care to patients in and around Greater Philadelphia (Montgomery and Chester County, PA)
  • Assessing patients' health through physical examinations, medical histories, and creating customized care plans
  • Communicating effectively with office staff and clinical operations across various internal platforms
  • Developing impressions, diagnoses, and treatment plans based on patient needs
  • Fostering trust and collaboration with patients, families, and caregivers during visits
  • Ordering diagnostic testing as medically necessary
  • Prescribing medications, supplies, and interventions as required

**Compensation Package:** $105,000 - $145,000+ per year (dependent on experience and qualifications)

Requirements

To be successful in this role, you should possess:

  • Board certification as a Nurse Practitioner in Pennsylvania
  • Strong clinical skills with the ability to accurately diagnose and treat patients
  • A positive demeanor and excellent people skills
  • The ability to problem-solve confidently and work independently
  • 1-2 years of primary care experience, preferably with geriatric patients (hospice experience a plus)
