Outpatient Care Coordinator

The Outpatient Navigator plays a vital role in the growth and development of our outpatient programs. This individual will identify, educate, and assist patients from our inpatient unit in transitioning to our outpatient services. Additionally, the Navigator will work to improve patient flow and access to care, while also conducting groups on inpatient units to discuss the importance of aftercare and step-down programs.
Responsibilities:
- Assist with the internal growth of outpatient programs
- Identify and educate patients for transition to outpatient services
- Improve patient flow and access to care
- Conduct groups on inpatient units to discuss aftercare and step-down programs
Requirements:
- MSW with LLMSW or LMSW
- 1-2 years of experience in a mental health setting, preferably in an inpatient psychiatric facility/unit
