The Licensed Practical Nurse (LPN) plays a vital role in the care coordination process at PACE Southeast Michigan.
As a member of the Interdisciplinary Team, the LPN works closely with participants, families, and healthcare providers to develop and implement individualized care plans.
The LPN is responsible for collecting and documenting assessment data, preparing and administering medications, and providing education to participants and their families.
Additionally, the LPN participates in the collection of information and maintains accurate records of participant functional status and care given.
The LPN advocates on behalf of participants and demonstrates accountability in resolving concerns or issues.
The LPN understands and promotes the Participant Bill of Rights and assesses and works toward achieving high levels of participant satisfaction.
The ideal candidate will have one year of experience working with frail or elderly populations and possess strong communication and interpersonal skills.
The LPN must be a Licensed Practical Nurse with current Michigan licensure and be medically cleared for communicable diseases.
Working conditions are variable with exposure to communicable diseases, due to patient environment.
The LPN must be able to establish and maintain interpersonal and interdisciplinary relationships and assume responsibility for self-development through continuing education.
The LPN must also participate in annual, mandatory in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training.
