Per Diem Ambulatory Care Associate

We are seeking a highly skilled and compassionate Per Diem Ambulatory Care Associate to join our team at Yale New Haven Health. As a key member of our interdisciplinary healthcare team, you will provide patient-centered care based on respect for patients' preferences, values, and needs.
Responsibilities- Patient Centered Care: Provide care and services with information regarding individual values, preferences, needs, and respect the patient or designee as a full partner in providing coordinated, compassionate, and culturally appropriate care.
- Communication: Interact effectively with patients, families, and all healthcare providers and employees, fostering mutual respect to enhance the patient experience and health outcomes.
- Team Collaboration: Function effectively within nursing and interdisciplinary teams, demonstrating open communication, mutual respect, team learning, and development.
- Quality and Safety: Minimize risk of harm to patients and providers through effective individual and team performance, practicing behaviors specific to High Reliability Organizations (HROs).
- Informatics: Use information and technology to communicate information, mitigate error, and support team decision-making.
- Professional Development: Demonstrate accountability for clinical competency in care delivery.
- A minimum of a high school diploma or GED.
- Graduation from an approved, accredited, postsecondary medical assisting program, Nursing Assistant Program, or Patient Care Associate Training.
- 12 months of experience prior work as an ACA I in an outpatient clinical setting or relevant patient care environment with documented competency demonstration.
- Preferred current medical assisting credential (certifications or registration) acceptable to, and recognized by, the Connecticut Department of Public Health.
Physical Demands: Able to lift a minimum of 40 pounds, perform tasks of physical endurance and dexterity as required by placement, and consistent use of proper body mechanics.
