Job Title: Outpatient RDN

Adventist Health is seeking a highly skilled and compassionate Outpatient RDN to join our team in Portland, Oregon.

About the Role:

We are looking for a dedicated and experienced Registered Dietitian Nutritionist to provide diabetes self-management education and training to patients with diabetes and their family members. The ideal candidate will have a strong background in nutrition and dietetics, with a focus on patient-centered care.

Responsibilities:
  • Provides diabetes self-management education and training to patients with diabetes and their family members.
  • Coordinates and monitors outpatient nutrition counseling services provided to outpatients/clients served.
  • Completes progress notes, serves as a liaison, and provides documentation in medical records to client's personal physician.
  • Develops and implements individualized or group, goal-directed diabetes management education plans for patients and their families.
  • Provides educational interventions that utilize interactive, collaborative, and skill-based training methods.
  • Works collaboratively with multi-disciplinary diabetes care teams to implement plans of care.
  • Performs nutrition assessments, reassessments, and patient/family education.
  • Provides medical nutrition therapy to outpatients with diagnoses other than diabetes.
Requirements:
  • Bachelor's Degree in food and nutrition or equivalent required.
  • Master's Degree preferred.
  • Experience as a clinical dietitian preferred.
  • Registered Dietitian (RD) certification required.
  • Certified Diabetes Educator (CDE) certification required.
