Palliative Care Nurse Practitioner

About the Role:
We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Gentiva Health Services. As a Palliative Care Nurse Practitioner, you will play a critical role in providing comprehensive, individualized care to patients with serious illnesses.
Key Responsibilities:
- Provide diagnosis, treatment, consultation, and follow-up care to patients referred to palliative care.
- Work collaboratively with the Palliative Care Medical Director and other team members to deliver high-quality care.
- Establish and maintain effective working relationships with patients, families, and healthcare teams.
- Communicate clinical findings and treatment plans to patients, families, and healthcare teams.
- Participate in quality improvement initiatives and maintain accurate records of patient care.
Requirements:
- Master's degree in Nursing from an accredited school of nursing.
- Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
- Current Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician agreeable to supervise work in palliative care program or must agree to enter a collaborative agreement with Palliative Medical Director, as required by state law.
- Part B Provider certification.
- Federal DEA license.
- State DEA license (if required).
