Position Overview:
The Vocational Nurse is tasked with delivering direct patient care under the guidance of a Registered Nurse.

Key responsibilities encompass adhering to the care plan, administering treatments, and collaborating with team members to achieve favorable patient care outcomes.

Key Responsibilities:
Deliver direct patient care as stipulated in the State Nurse Practice Act.

Execute current nursing practices following patient assessments and approved care plans initiated by the Registered Nurse.

Ensure accurate and timely documentation of patient services in alignment with the care plan.

Provide education and information to patients and their families/caregivers regarding diagnoses and self-care plans.

Assist in coordinating hospice services, reporting identified needs to interdisciplinary group members such as hospice aides, occupational therapists, physical therapists, medical social workers, speech therapists, dietitians, or hospice managers.

Utilize equipment and supplies effectively and efficiently.

Administer appropriate pain and symptom management while evaluating patient responses to treatments and medications.

Maintain knowledge of state, federal, local, and accreditation regulations governing hospice service delivery.

Engage in personal and professional development, as well as participate in the organization's quality management initiatives.

Meet daily productivity standards established by the agency.

Perform additional duties as assigned by the Case Manager and Hospice Manager.

Actively engage in the Quality Management Program.

All clinical staff are expected to participate in variable shifts, including weekend rotations as needed by staffing requirements and departmental holiday rotations.

Qualifications:
Current and active California Licensed Vocational Nurse (LVN) license.

Minimum of one year of experience required; three years preferred.

Experience in community health/hospice or medical/surgical settings is preferred.

Current CPR certification.

Valid driver's license with an insured vehicle, in accordance with state and/or organizational requirements.

Adherence to accepted professional standards and practices.

Strong verbal and written communication skills, along with organizational abilities.

Understanding of the hospice philosophy and the needs of terminally ill patients.

Bilingual in Spanish is preferred but not mandatory.

Physical Requirements:
Ability to lift up to 50 lbs.

Ability to push/pull up to 50 lbs.

Ability to stoop, bend, and squat.

Ability to walk up one flight of stairs.

Ability to walk one mile.

Ability to sit for a minimum of one hour.

Visual and auditory acuity.
