Position Summary:
The Licensed Vocational Nurse (LVN) is essential in providing direct patient care under the guidance of a Registered Nurse. Key responsibilities include adhering to care plans, administering treatments, and collaborating with the healthcare team to achieve optimal patient outcomes. Core Responsibilities:
  • Deliver direct patient care in accordance with the State Nurse Practice Act.
  • Execute nursing practices based on patient evaluations and approved care plans established by the Registered Nurse.
  • Maintain accurate and timely documentation of patient services in alignment with the care plan.
  • Educate patients and their families/caregivers regarding relevant diagnoses and self-care strategies.
  • Assist in coordinating hospice services, communicating needs to interdisciplinary team members as necessary.
  • Utilize equipment and supplies efficiently.
  • Administer effective pain and symptom management while assessing patient responses to treatments.
  • Demonstrate understanding of regulations governing hospice service delivery.
  • Engage in personal and professional growth and participate in the organization's quality management initiatives.
  • Meet daily productivity expectations set by the agency.
  • Perform additional responsibilities as assigned by the Case Manager and Hospice Manager.
  • Actively contribute to the Quality Management Program.
All clinical staff may be required to work flexible shifts, including weekends, based on staffing requirements and departmental holiday schedules.

Qualifications:
  • Active California Licensed Vocational Nurse (LVN) license.
  • At least one year of experience; three years preferred.
  • Experience in community health/hospice or medical/surgical environments is advantageous.
  • Current CPR certification.
  • Licensed driver with insured vehicle, in accordance with state and/or organizational requirements.
  • Commitment to professional standards and practices.
  • Strong verbal and written communication skills along with organizational capabilities.
  • Understanding of hospice philosophy and the needs of terminally ill patients.
  • Proficiency in Spanish is a plus.
Physical Requirements:
  • Ability to lift and push/pull 50 lbs.
  • Capability to stoop, bend, squat, and navigate stairs.
  • Ability to walk distances and sit for extended periods.
  • Visual and auditory acuity.
