We are seeking a skilled and compassionate Hospice Registered Nurse (RN) to join our team as a RN Case Manager. In this role, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and ensuring seamless transitions of care.

Key Responsibilities
  • Provide direct patient care services, including assessments, care planning, and implementation of individualized plans of care.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive care.
  • Conduct patient and family assessments to identify needs and develop effective care plans.
  • Manage and coordinate care transitions, ensuring continuity and quality of care.

Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
