Job Summary:

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at St. John's Hospice. As a Hospice Care Specialist, you will provide high-quality care to patients in their homes, promoting their physical, emotional, and spiritual well-being.

Key Responsibilities:

Manage a caseload of patients, providing individualized care and support
Develop and implement personalized care plans, working closely with patients, families, and healthcare teams
Provide emotional support and companionship to patients, enhancing their quality of life
Collaborate with healthcare professionals to ensure seamless care coordination
Maintain accurate records and reports, adhering to regulatory requirements

Requirements:

Current CNA certification
High School Diploma or equivalent
Minimum 1 year of experience in healthcare, preferably in hospice care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
COVID-19 vaccination required
