Position Title: Patient Care Advocate and Coordinator

We are looking for a compassionate and committed healthcare professional to join our organization as a Patient Care Advocate and Coordinator. In this role, you will assist members in navigating the complexities of the healthcare system, ensuring they receive the necessary support and guidance.

Key Responsibilities:

  • Engage with members to provide education on preventive health measures.
  • Distribute educational resources and materials to enhance member knowledge.
  • Inform members about various wellness programs and initiatives.
  • Organize and manage healthcare appointments for members, including visits to medical professionals, dental care, and specialists.
  • Assist members in setting up remote monitoring tools such as blood pressure monitors, glucose meters, and other health devices, ensuring they understand their usage.
  • Coordinate the delivery of necessary supplies for health devices on a monthly basis.
  • Assess home safety for members needing modifications, facilitating necessary changes when applicable.
  • Arrange meal services for eligible members recently discharged from healthcare facilities.
  • Identify community resources for members requiring transportation and other support services.
  • Provide assistance in finding home care solutions, collaborating with preferred service providers.
  • Support members in understanding their benefits and coordinating with plan vendors.
  • Follow up with members to ensure ongoing support and availability for their needs.
  • Develop outreach initiatives to communicate available benefits and services to members.
  • Participate in client meetings to share information about services and provide educational materials.
  • Work closely with clients to address any overlooked needs of members.
  • Focus outreach efforts on members identified by management as needing additional assistance.
  • Review pre-certification alerts and ensure members are informed about provider ratings.
  • Conduct follow-up visits with members post-hospital discharge.
  • Perform on-site visits to members who opt for this service.
  • Ensure coordination of third-party support services for members as needed.
  • Collaborate with pharmacists to provide medication reconciliation upon request.
  • Coordinate social worker services when requested by members.
  • Maintain a professional demeanor at all times.
  • Uphold confidentiality and adhere to PHI and HIPAA regulations consistently.
  • Show respect and empathy towards all members.
  • Collaborate with colleagues and management to review and enhance existing processes, ensuring high standards of service delivery.
  • Communicate regularly with HR and clients to address any emerging concerns or suggestions.
  • Engage with healthcare providers to ensure comprehensive member support.
  • Travel to member locations and events as required.
  • Perform additional duties as assigned by management.

Qualifications:

  • Current licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), or Medical Assistant (MA); inactive licenses will also be considered.
  • A minimum of three years of experience in healthcare coordination or a related support role.
  • Exceptional communication and interpersonal skills, with a strong ability to provide emotional support.
  • Excellent organizational skills and attention to detail, capable of managing appointments and maintaining accurate records.
  • A humble and approachable demeanor, genuinely interested in assisting others.
  • Ability to work independently while effectively collaborating with healthcare professionals.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint.
