Key Responsibilities:
- Actively participate as a vital member of the healthcare team, ensuring safety, adherence to best practices, and high-quality care standards throughout the continuum.
- Coordinate a variety of self-management support and disease registry initiatives for the targeted population.
- Utilize data analytics to assist in managing the health outcomes of the identified group.
- Engage closely with physicians, administrative personnel, and staff across all facilities to review medical records, provide insights on quality-related incidents, and present data to support care quality improvements.
Qualifications:
- Education: A valid Registered Nurse license is required; a Bachelor of Science in Nursing (BSN) is preferred.
- Experience: A minimum of three (3) years of experience in a hospital setting is required.
- Licensure: Must possess a current RN license in Washington State.
- Additional Skills: Bilingual capabilities, particularly in English and Spanish, are preferred. The ability to engage with a culturally diverse patient population is essential.
Position Classification: This role is categorized as safety-sensitive.
Compensation: Salary range starts at $54.03 per hour.
