Job Title: Social Worker - Home Health & Hospice

At CommonSpirit Health, we are committed to delivering compassionate, high-quality care to our patients and their families. We are seeking a skilled Social Worker to join our Home Health and Hospice team.

Job Summary:

The Social Worker will be responsible for providing social work services to patients and their families in the home health and hospice setting. This includes assessing the psychosocial status of clients, developing and implementing social work plans, and providing information and referral services to clients and families.

Responsibilities:
  • Assess the psychosocial status of clients and communicate findings to the registered nurse.
  • Develop and implement social work plans to address the needs of clients and their families.
  • Provide information and referral services to clients and families regarding environmental needs and community resources.
  • Market and promote agency programs and events in the local community.
  • Serve as liaison between clients and families and community agencies.
  • Maintain collaborative relationships with organization personnel to support client care.
  • Participate in the development of the total plan of care and case conferences as required.
  • Assist medical team in understanding significant social and emotional factors related to health problems.
  • Participate in discharge planning.
  • Coordinate new volunteer training, continued education, and annual evaluations.
  • Assign volunteers to each hospice family and coordinate visits.
  • Coordinate the bereavement program.
  • Assure proper documentation of social services in clinical records in compliance with state, federal, and Joint Commission standards.
Qualifications:
  • Bachelor's degree in Social Work.
  • Licensed as a Social Worker in Minnesota and North Dakota.
  • One year experience in healthcare.
  • Excellent verbal, written communication skills and organizational skills.
  • Basic computer skills.
