Overview: The Registered Nurse (RN) Care Manager for the Patient Aligned Care Team (PACT) plays a crucial role in delivering high-quality care to Veterans. Reporting to the Nurse Manager for Primary Care Services, this position emphasizes professional conduct and collaboration across various levels of the Veterans Affairs Health Care System.

Key Responsibilities:

  • Deliver comprehensive nursing care as guided by the Nurse Manager, aligning with the mission and vision of the Healthcare System.
  • Address acute and chronic health needs of Veterans, focusing on adult and elderly populations.
  • Utilize the nursing process and evidence-based practices to work with the interdisciplinary team, including patients, providers, and support staff, to create individualized care plans.

Essential Duties:

  • Provide competent, evidence-based care tailored to the unique needs of primary care patients.
  • Oversee care coordination and manage assigned patient panels, ensuring timely evaluations and access to necessary services.
  • Engage in health promotion and education, guiding patients in wellness, disease prevention, and chronic disease management.
  • Foster a supportive environment for healthcare professionals, encouraging ongoing education and quality improvement initiatives.
  • Collaborate with internal and external services to facilitate seamless transitions of care for patients.
  • Empower patients and families through education focused on self-management and shared decision-making.

Qualifications:

  • Must possess a full and unrestricted nursing license from any U.S. state or territory.
  • Demonstrated proficiency in English, both written and spoken.
  • Graduate of an accredited nursing program.

Work Schedule: This position operates Monday through Friday, from 8:00 AM to 4:30 PM. Telework options are not available for this role.
