Key Responsibilities:

  • Deliver competent, evidence-based, gender-specific care to primary care patients while supervising licensed vocational nurses and nursing assistants as necessary.
  • Provide direct care and coordination for Veterans within the Primary Care Department, managing assigned patient panels and ensuring appropriate access to care.
  • Implement health promotion and education initiatives, focusing on wellness, disease prevention, and chronic care management.
  • Lead nursing education and quality improvement efforts, fostering a supportive environment for professional development.
  • Collaborate with internal and external services to facilitate effective care transitions, ensuring an interdisciplinary approach to patient needs.
  • Educate patients and families on self-management, shared decision-making, and wellness, tailored to individual goals.
  • Utilize research-based knowledge and standardized protocols to enhance patient engagement and determine care management requirements.

Physical requirements include the ability to perform tasks such as standing, walking, and transferring patients, as well as compliance with health screening protocols.
