Care Management is a collaborative practice model that includes patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. As a Care Manager RN at St Joseph Hospital, you will play a vital role in facilitating care along a continuum through effective resource coordination.

Key Responsibilities

  • Assess patient needs and develop individualized plans of care
  • Coordinate care with healthcare providers, social services, and community resources
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Advocate for patients and their families to ensure access to necessary resources and services

Requirements

  • Graduate of an accredited school of nursing
  • California Registered Nurse License upon hire
  • 3 years of clinical experience in an HMO, medical group, affiliated model, hospital, or medical office/clinic setting

Preferred Qualifications

  • Bachelor's Degree in Nursing or Master's Degree in a related field
  • Certification in Case Management (CCM, ACM) upon hire
  • Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire
  • 3 years of experience in utilization management or case management
