Job Title: Enrollment Registered Nurse

High Desert PACE is seeking a skilled and compassionate Enrollment Registered Nurse to join our team. As a key member of our outreach and enrollment team, you will work closely with our Business Development Representatives and Enrollment Representatives to determine the eligibility of future participants for our High Desert PACE Program.

Responsibilities:
  • Conduct home visits and/or center-based clinic appointments to assess the level of care required for prospective PACE participants.
  • Collaborate with the interdisciplinary team to determine eligibility and develop care plans.
  • Work closely with the California Department of Health Care Services (DHCS) PACE Nurse Assessor to make eligibility decisions.
  • Manage semi-annual audits of participants' medical records and complete semi-annual level of care redetermination for all participants.
  • Support business development and census growth by participating in outreach programs and collaborating with the Business Development Department.
Requirements:
  • Associate degree from an accredited nursing school.
  • Current Registered Nurse license in the state where working.
  • Current CPR certification or the ability to obtain CPR certification.
  • 3 years of healthcare experience with an emphasis in geriatrics.
  • 1 year of experience working with the frail or elderly.
