ROLE SUMMARY

The primary responsibility of the Therapy Services Coordinator is to facilitate the daily operations of rehabilitation services, which includes organizing home health visits and managing administrative functions to ensure effective patient care delivery. There may be occasions where direct patient interaction is required.

KEY RESPONSIBILITIES:

1. Aids the Clinical Therapy Manager with daily administrative duties such as reviewing patient referrals, conducting care audits, scheduling therapy sessions, managing patient documentation, and liaising with healthcare professionals.

2. Organizes and coordinates home health visits, ensuring prompt and effective delivery of therapy services to clients.

3. Acts as a liaison for patients, caregivers, and healthcare providers, addressing questions and coordinating care strategies.

4. Assists therapy personnel by preparing essential documentation, monitoring visit schedules, and ensuring adherence to regulatory standards.

5. Collaborates with the therapy team to enhance patient outcomes, refine processes, and improve care quality.

6. Engages in quality enhancement initiatives and contributes to the ongoing improvement of therapy services.

7. Upholds patient confidentiality and adheres to ethical and professional standards in all interactions.

8. Provides direct physical therapy services as directed by the supervising physical therapist and in accordance with the physician's care plan.

9. Participates in community outreach and public relations activities that highlight the organization's role in healthcare delivery.

10. Performs additional tasks and responsibilities as assigned, adapting to departmental needs to support overall operations and ensure exceptional service quality.

QUALIFICATIONS:

1. Graduate of an accredited Physical Therapy Assistant program.

2. Current certification or licensure as mandated by state regulations.

3. Maintains up-to-date CPR certification.

4. Excellent communication, organizational, and problem-solving abilities.

5. Valid driver's license with an insured vehicle that meets state and organizational requirements and is in good condition.

6. Compassionate demeanor, empathy for patients, and a commitment to providing high-quality care.
