Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Chillicothe, Ohio. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary:

We are looking for a compassionate and dedicated Occupational Therapist to work with our interdisciplinary team. The successful candidate will have a current license in the state of employment, a minimum of one year of experience as an OT in an acute care setting, and valid driver's license and auto insurance. The ideal candidate will be able to work independently and as part of a team, with excellent communication and interpersonal skills.

Responsibilities:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current license in the state of employment.
  • Minimum one year of experience as an OT in an acute care setting.
  • Valid driver's license and auto insurance.
