Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence St. Peter Hospital. As a Clinical Pharmacist, you will play a vital role in providing pharmaceutical care to our patients, working collaboratively with the healthcare team to ensure optimal drug therapy outcomes.
Key Responsibilities- Provide pharmaceutical care to patients, including medication therapy management and patient education.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Conduct medication use evaluations and make recommendations for improvement.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest advancements in pharmacy practice and apply this knowledge to improve patient care.
- Graduation from an accredited college of pharmacy.
- Washington Pharmacist License.
- Completion of an ASHP accredited residency program (preferred).
- One year of hospital pharmacy experience (preferred).
