Nurse Care Coordinator for Developmental Disabilities

Eliot Community Human Services is committed to providing exceptional support services for adults with developmental disabilities and brain injuries. As a Nurse Care Coordinator, you will play a vital role in ensuring the health and well-being of our clients.
Responsibilities:
• Provide education and consultation on medical issues, treatments, and medications to individuals and staff
• Conduct skills trainings and group training sessions as needed
• Review medical documentation, complete assessments, and prepare documentation for new referrals and discharges
• Perform direct care and medical support services as needed
• Hire, train, and supervise staff in Brain Injury programs
• Meet with managers and staff to provide training and consultation on new medical diagnoses, medications, diets, and protocols
• Develop treatment goals related to medical, medication, or health concerns
• Coordinate medical care with community providers and VNA services
Requirements:
• 3 years of experience in a community health setting
• Expert communication and computer software skills
• Expert medical administration skills
• Bachelor's degree in Nursing (or higher) preferred
• RN licensure preferred
