Nurse Coordinator for Adults with Developmental Disabilities

We are seeking an experienced LPN to coordinate residential services supporting these adults. The ideal candidate will have 3 years of related experience and the skills to establish supportive relationships with individuals with developmental disabilities and brain injuries.
The successful candidate will be responsible for:
- Providing education and consultation to individuals and staff on medical issues, treatments, medications, wellness, and prevention
- Establishing individualized training plans and conducting group training sessions as needed
- Reviewing medical documentation, completing medical assessments, and preparing medical documentation for new referrals and discharges
- Performing direct care and medical support services as needed
- Hiring, training, and supervising staff in Brain Injury programs
- Coordinating medical care with community providers and VNA-related services
