We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our nursing team at our Pipestone facility. As a member of our interdisciplinary care team, you will provide high-quality nursing care to our residents, working collaboratively with our healthcare professionals to ensure the best possible outcomes.
Job SummaryThe LPN provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role requires a strong commitment to delivering exceptional patient care, with a focus on safety, quality, and compassion.
Responsibilities- Provides direct patient care, including administering medications, treatments, and performing various medical procedures.
- Conducts thorough assessments of patients, identifying their physical and emotional needs, and develops individualized care plans to address these needs.
- Collaborates with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Communicates effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
- Participates in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
To be successful in this role, you will need:
- A graduate degree from an accredited practical nursing program as a Licensed Practical Nurse.
- An unencumbered LPN license with the State Nursing Board and/or multi-state licensure privileges.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Excellent communication and interpersonal skills, with a focus on empathy and compassion.
- Ability to maintain confidentiality and handle sensitive information with discretion.
