We are seeking a highly skilled Registered Nurse (RN) to join our team as a Care Manager. This role requires strong clinical judgment, effective communication, and a commitment to delivering high-quality patient care.
Key Responsibilities- Prescribe, delegate, and coordinate patient care using clinical judgment and evidence-based guidelines.
- Develop and implement an appropriate plan of care for patients, promoting best outcomes and ensuring patient safety, effectiveness, and patient rights.
- Oversee care delivered by the patient care team, coordinate the plan of care, and provide education and learning opportunities for patients, families, and the patient care team.
- Collaborate with physicians, families, and other healthcare professionals to develop and implement an appropriate plan of care, promoting patient-centered care and best outcomes.
- Advocate for patients, represent their concerns, and identify and resolve ethical and clinical concerns.
- Deliver care with a team-oriented approach, emphasizing good customer relations, sound clinical judgment, and appropriate decision-making abilities.
- At least 4 years of experience in pediatric nursing, case management, care management, care coordination, utilization review, or community-based nursing.
- BSN degree.
- Registered Nurse in the State of Texas upon hire.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) certification preferred.
