Job Summary

Mental Health Partners is seeking a skilled Housing Case Manager to join our team. As a Housing Case Manager, you will work closely with clients to develop and implement individualized case plans, connecting them with essential resources and services.

Key Responsibilities
  • Develop and implement case plans that address the unique needs of clients, including housing, healthcare, and social services.
  • Build and maintain relationships with community partners, including health homes, crisis centers, and withdrawal management services.
  • Provide education and support to clients on topics such as housing options, benefits navigation, and health and wellness.
  • Collaborate with internal teams to ensure seamless service delivery and effective case management.
  • Document client progress and outcomes, using data to inform program development and improvement.
Requirements
  • Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor's degree.
  • Experience as a Case Manager, preferably in an outpatient setting.
  • Current CO Driver's License and a safe driving record.
  • Ability to travel to different locations, with occasional overnight stays.
