Job Summary:

The Primary Care Provider is a key member of the InnovAge team, responsible for providing high-quality primary care to our participants. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.

Key Responsibilities:

  • Evaluation and Treatment: Conduct thorough evaluations of participants, documenting information in their charts and providing appropriate treatment plans.
  • Diagnostic Testing: Order and interpret diagnostic tests, including blood draws and EKGs, to inform treatment decisions.
  • Nursing Interventions: Perform dressing changes, administer medications, and provide other necessary nursing interventions as needed.
  • Treatment Planning: Formulate treatment plans, including writing prescriptions and providing ongoing follow-up care, to ensure optimal health outcomes.
  • Education and Support: Educate staff, participants, and caregivers on healthcare issues and treatments, providing emotional and spiritual support as needed.
  • Care Coordination: Collaborate with other primary care providers to manage acute and long-term medical needs, overseeing care in various settings, including nursing homes, homes, and group clinics.
  • Documentation: Ensure accurate and timely documentation of medical care in participant records, adhering to InnovAge's documentation policies.

Requirements:

  • Licensure: Current State-issued Nurse Practitioner license
  • Experience: 1+ year as a practicing Nurse Practitioner
  • Certification: DEA Certification with full prescriptive authority
  • Background: 5+ years as a Registered Nurse with experience in elderly care

Preferred Qualifications:

  • Language Skills: Bi-lingual
