The Primary Care Physician (PCP) is a licensed and Board Certified/Board Eligible professional specializing in internal or family medicine. This role is integral to our clinical operations team, delivering direct patient care primarily within an ambulatory healthcare environment, and occasionally in acute care settings, nursing homes, skilled nursing facilities (SNF), and home care, depending on the assignment's requirements.


Key Responsibilities:

Conduct comprehensive geriatric assessments, gather medical histories, perform physical examinations, diagnose and treat conditions, develop care plans, provide health education, and facilitate specialty and case management referrals. Ensure thorough documentation that aligns with ChenMed's standards for quality, service, productivity, and teamwork.

The PCP must exhibit the capability to work autonomously as well as collaboratively with other healthcare professionals.

Engage with relevant managers and medical directors to maintain compliance with established guidelines while participating in risk and quality management initiatives, clinical meetings, and other necessary gatherings.

Adhere to departmental objectives, performance standards, regulatory compliance, and policies to ensure quality patient care.

Function independently as a primary care provider within a patient care team, performing and documenting physical assessments and patient histories, analyzing patient condition trends, and developing, documenting, and implementing management plans based on collected data.

Responsibilities also include writing admission, transfer, and discharge orders, and referring patients for consultations in various specialties as needed, such as dermatology, neurology, and psychiatry. Recognize situations requiring immediate intervention and initiate life-saving measures when necessary.

Collaborate with multidisciplinary team members to assess and enhance current healthcare delivery systems, identifying and implementing new practice patterns as appropriate.

Work closely with physicians, nurses, physical therapists, social workers, families, and key caregivers to facilitate timely transitions to lower levels of care when medically suitable.

Introduce yourself to patients and their families, clarifying the role of the primary care provider.

Foster a collaborative relationship to empower patients and families in making informed decisions regarding care goals, including palliative and hospice options.

Manage resource utilization and reimbursement for services effectively.

Board certification in Internal Medicine or Family Medicine is preferred, with a requirement to achieve Board certification within two years. A minimum of one year of clinical experience in a geriatric, adult, or family practice setting is preferred.
