Job Summary

The Senior Patient Care Coordinator is responsible for coordinating, planning, and administering various administrative management, financial, and operational duties in collaboration with the Administrative Nursing Supervisor, Nurse Manager, and Assistant Vice President of Specialty Nursing Division.

Estimated Salary: $65,000 - $90,000 per year
This salary range is based on industry standards and location in Petersburg, NY, United States.

Key Responsibilities
• Coordinate patient access, communication, and transitions throughout the acute care stay
• Collaborate with other service and department leaders towards attainment of short- and long-term goals and objectives
• Provide direction and support to unit-based staff
• Assist in the development, implementation, and evaluation of policies, programs, and services consistent with the hospital's mission and departmental/unit vision and philosophy
• Participate in weekend coverage and off-shift assignments as needed

About the Job

Requirements
We are looking for a highly motivated and organized individual who can work effectively in a fast-paced environment. The ideal candidate will have excellent communication skills, both written and verbal, and be able to work collaboratively with multiple stakeholders.
