About the Role

As a Clinical Care Coordinator at JobRialto, you will play a critical role in ensuring the highest level of care for our patients. This involves coordinating and implementing patient care plans that are tailored to the unique needs of each individual. You will work closely with healthcare teams to ensure seamless transitions and efficient care delivery.

Responsibilities

  1. Develop and implement patient care plans that address the specific needs of each patient.
  2. Collaborate with healthcare teams to ensure timely and effective care delivery.
  3. Maintain accurate and up-to-date patient records.
  4. Respond promptly to emergencies and schedule changes.
  5. Ensure a safe and clean working environment by adhering to infection control protocols.

Requirements

  • Graduate of an accredited Registered Nursing program.
  • Current New Mexico RN license or a valid Compact state license.
  • BLS certification is required within two weeks of hire.
  • All other required certifications must be obtained within six months of hire and maintained thereafter.
